How we collaborate

CHC provides both individual and group counseling to the three buildings at the Anoka County Juvenile Center five days a week. In addition, our team provide staff training and consultation.

Alexandra House operates the only 24-hour domestic and sexual violence emergency shelter in Anoka County.  CHC provides in-house therapy to within the shelter as well as working with survivors who have transitioned into the community.

This program supports families, couples, and youth after county social workers identify additional therapeutic resources as a benefit to stabilization. Anoka County strives to support its families with no cost resources, allowing clients the space to gain support and education for the difficulties present in their lives.

In service to those seeking county services, but need an expedited diagnostic assessment, CHC has partnered with Anoka County, in both the Children’s and Adult’s Mental Health Services. Through pairing with case managers, we provide assessment interviews and the subsequent documentation to allow for services to be accessible more quickly. Case managers work together with CHC to provide all available information so as to remove barriers for children and adults whose assessments have expired, or are nonexistent. This extends to youth in shelter at hospitals, those who are in hardship, and families needing extra support. A CHC therapist works with the client and case manager to identify diagnosis and services most beneficial to ongoing care. In some cases, therapy within CHC may be available, or additional referrals to county programs.

Partnership with the St. Francis Schools reaches across elementary, middle and high school buildings, thanks to the proactive mental health lens of this district. Taking care of their students elicited placement of therapists in the buildings of the school during the school day, lessening strain on families and creating more ease and accessibility for youth to have mental health care. Families work together with CHC and the schools, using their insurance normally, to align services. The flexibility offered through this option is highlighted by the ability to extend services beyond the school and into the clinic if needed, such as during holiday and summer breaks. This collaboration normalizes mental health for all ages and specifically speaks to our value of suicide prevention.
